Creating a form is done by adding the Form block to a page. The form block shows a normal form which the user can fill in and send to the system (and can also automatically be emailed to you).

When adding a question to the form, 3 options have to be entered:

• Question
  • Name of the question. Enter the question which the user will see here. i.e. "Contact Name:"
• Answer Type
  • The following options are possible:
    • Text Field
    • Text Area – Bigger Text Field
      • Here 2 more options are available:
        • Width – Width of the text area in pixels
        • Height – Height of the text area in pixels
    • Radio button
      • Here more options are available:
        • A text area will appear where on every line a possible answer can be entered.
    • Select box – Dropdown box
      • Here more options are available:
        • A text area will appear where on every line a possible answer can be entered.
    • Checkbox list
      • Here more options are available:
        • A text area will appear where on every line a possible answer can be entered.
    • File upload – Select a file which will be uploaded.
• This question is required
  • When this checkbox is ticked this question has to be answered in order to submit the form.

It is possible to save the form at any time by pressing the "Update" button. Clicking on the "Cancel" button will discard any changes made to the form.

Open the form for editing, by clicking on it while in edit mode. The same editor will appear and can be used to change the questions in the form. Use the edit tab to edit or delete existing questions. While editing the question you will get to a similar editor as the create question screen.

It is possible to save the form at any time by pressing the "Update" button. Clicking on the "Cancel" button will discard any changes made to the form.

In this tab it is possible to preview the form as it will appear on the page. You cannot perform any other actions here.

In the options tab it's possible to set a number of options that matter for how the form reacts to the user.

• Form name
  • Name of the form how it appears in the reports section in the dashboard. Use an easy recognizable name.
• Message to display when completed
  • What will the user see as a “Thank you” message after he submits the form?
• Notify me by email when people submit this form
  • Enter email addresses here where the confirmations messages will be send to. Multiple addresses can be entered in the text field that appears after ticking the checkbox. Use a comma, to separate the addresses.
• Solving a CAPTCHA Required to Post?
  • Does the user have to complete a CAPTCHA text/word to be able to submit a form? This way the system will know it’s not entered by a computer, but by a real human being.
